Request Sacramental Records

All requests for sacramental records must be made in writing by letter or email to Cameron Engelbrecht, Coordinator of Faith Formation ([email protected]). Requests can only be granted if requested by the individual to whom the record belongs, the parent or guardian of a minor to whom the record belongs, or a Catholic parish or other religious institution on behalf of the person. 

All requests must include the following information: the full legal name of the person, the date of birth, the approximate year of reception of the sacrament, and the type of record (baptismal, first holy communion, etc.) needed.
